When you contact us or request a consultation, we may collect your name, telephone number, email address, the postal code where care is needed, your preferred contact time and information you choose to provide about your inquiry.
Please do not include diagnoses, medical records or detailed medical information in the website inquiry form. Information needed for a care assessment should be discussed through the appropriate intake process.
Why we use personal information
We use inquiry information to respond to requests, discuss available services, arrange consultations and coordinate follow-up. Where you proceed with care, additional information may be needed to assess needs, plan services, communicate with authorized people and administer the care relationship.
Consent and choice
By submitting a care inquiry, you agree that TLC may contact you about that inquiry. This does not automatically enrol you in promotional communications.
Where consent is required for another use of your information, TLC will explain that use and request the appropriate consent. You may contact us about withdrawing consent, subject to applicable requirements and any effect this may have on services we can provide.
Sharing information
Access to personal information is limited to people who need it for their authorized responsibilities. Information may be shared with service providers supporting TLC’s operations where necessary for those services, subject to appropriate confidentiality and information-handling requirements.
Information may also be disclosed with your consent or where required or permitted by applicable law.
TLC does not sell personal information.
Website analytics and advertising measurement
Our website uses technology provided by Google and Ploy to support website operation and analytics. Depending on the technology and settings in use, these services may process information such as IP addresses, browser and device information, page visits and interactions.
The home-care landing page also provides a separate choice about optional Meta advertising measurement. That choice applies to Meta measurement; it does not itself control Google or Ploy services.
Care-inquiry answers and detailed medical information should not be included in advertising measurement events.
Safeguarding information
TLC uses administrative, physical and technical safeguards appropriate to the information it handles. Access is restricted to authorized people who require the information for their responsibilities.
Retention
Personal information is retained for as long as needed for the purposes for which it was collected and to meet applicable obligations. When it is no longer required, it is securely destroyed or otherwise handled appropriately.
Access and correction
You may request access to your personal information or ask us to correct inaccurate information. We may need to verify your identity before responding. Where access cannot be provided, we will explain the reason as appropriate.
