What is Family-Managed Home Care?

Family-Managed Home Care is a government-funded program in Ontario that allows families to take a more active role in managing their care services.

How It Works

Step 1

Start with a Conversation

Connect with our FMHC Coordinator to discuss your situation, your goals, and how the program works. We can help you navigate the application process for obtaining FMHC funding.

Step 2

Apply for FMHC Funding (if you have not already applied)

Applications are through Ontario Health at Home. (OHaH). You must submit a referral. Anyone can make a referral to OHaH on your behalf and with your consent, a family doctor, friend, family member, caregiver, neighbour and even you, yourself.

Step 3

In-Home Assessment

We complete a personalized FREE in-home assessment to understand your loved one’s medical, personal, and daily care needs.

Step 4

Matched with a Dedicated Care Team

You are matched with a consistent Primary Nurse or PSW who understands your routine and preferences.

We also assign Backup Caregivers so you are never left without support during sick days, vacations, or unexpected changes.

Step 5

Ongoing Support & Communication

We stay connected through ongoing monitoring and communication, ensuring continuity of care and the ability to adjust as needs evolve.
